What We Do:
Meeting with the patient and the family – A complimentary assessment is provided for every prospective client
Home inspection – Covering all areas of living to ensure safety
Create a Plan of Care – Customized to fill the need of every individual patient
Fill the Need – The ideal caregiver (CNA, LVN, RN) is carefully selected from the hundreds on our roster based on the need. People who have had a stroke may have trouble with many activities that were easy before, such as walking, talking, and taking care of “activities of daily living” (ADLs). These include basic tasks such as bathing, dressing, eating, and using the toilet, as well as more complex tasks called “instrumental activities of daily living” (IADLs), such as housekeeping, using the telephone and driving to doctor’s appointments
Care Giver Training – We will bring the caregiver to your facility for training to ensure a safe transition to your home
Start Up – A supervisor or the Director of Nursing will be at your home and review and implement the plan of care
Supervisory Visits – Random visits are completed and every 60 days to adjust the plan of care as needed
Peace of Mind – People who have had a stroke have an increased risk of another stroke, especially during the first year after the original stroke. The risk of another stroke goes up with older age. While some risk factors for stroke (such as age) cannot be changed, the risk factors for the others can be reduced through use of medicines or changes in lifestyle. We will work with your doctor and the patient to make healthy changes in the patient’s lifestyle.
